There are a number of nude cruises available from eg. Bare Necessities (
www.cruisebare.com) and it really depends what you like. If you enjoy cruising on a large cruise liner with 3000 other like minded people around the carribean, this could be for you. Everyone can be nude on board as soon as the ship has left port and dress only for meals.
We are not fans of the large cruise ships but prefer the smaller (only 200 passengers) sailing ship the Royal Clipper which Bare Necessities charter for a nude sailing trip annually. It gets booked up very quickly! Again, you can be nude except when in port or during mealtimes.
In both cases, the operator offers (dressed) shore excursions - although in many cases there is an option to take a trip to a local naturist beach if there is one nearby.

One note of caution : Some companies also offer "nude" cruises targeting the swinging lifestyle. That's perfectly fine if that's what you are looking for - not so much if you thought it was just a naturist cruise.
